Had a chance to update your Patient ID Card lately? Now you can, just read below for instructions on what to do.

Try our new online site to make your Patient ID Card renewal easier and convenient, rather then visiting us in person this year. First have you obtained your new updated Doctor's recommendation yet? If not you need to get this step done first to update with our program and receive a new ID card. You can call us for a Doctor referral or go back to the last Doctor you visited with last year.

If you have a new Doctor's note proceed to update information and upload a copy of the note and your California Drivers License or California ID. Simply do this by clicking "My Patient Profile" tab on the top left side of this page. Once there you just need to update your information on file and then upload your current copy of your note and Driver License or ID from the California DMV. When that is completed just click on the tab at the bottom to save and proceed to the secure payment portal to pay the $30.00 Membership renewal fee.
